Applying for Services

Gateways Community Services: Applying for ServicesIn order to access services provided by Gateways Community Services, individuals must provide information and meet one-on-one with an intake specialist to determine if they are eligible. To initiate the process, you must complete the following steps prior to scheduling your appointment with the Intake Specialist.

Please be aware that the completion of the Request for Eligibility Determination form and the granting of an intake appointment are not a guarantee of eligibility or a promise that services will be provided.

Step One:

Gather information which shows that the applicant has a developmental disability or Acquired Brain Disorder. See document list below:

Step Two:

Complete the Request for Eligibility Determination Form available here (English / Spanish).

Step Three:

Contact the Intake Specialist at (603) 459-2787, or by e-mail at to schedule your one-on-one, once you have completed steps one and two above.

Step Four:

Attend your Intake Appointment. Plan to spend at least one and a half hours to complete the intake appointment. During this appointment, the Request for Eligibility Determination Form and required documents listed under Step One will be reviewed. A functional assessment tool will also be completed with the applicant and an individual who is most familiar with the applicant's functionality.

Can I e-mail you the Eligibility Form and start getting services right away?

No. Applicants must provide all of the information outlined above and schedule an appointment to apply for services. Please be aware that the completion of the Request for Eligibility Determination form and the granting of an intake appointment are not a guarantee of eligibility or a promise that services will be provided.

Do I need Medicaid to be eligible for your services?

Medicaid is not required for eligibility. However, families are strongly encouraged to apply for Medicaid.
